Triage Nurse – OSS (Kano) at Society for Family Health (SFH)

Job Profile: 

The Triage Nurse will also serve as the Medical Records Officer and is responsible for managing client records; ensuring accurate data entry, filing/archiving and retrievals.

Job-role: The successful candidate will perform the following functions:

  • Clinical Services: Nursing interventions (e.g. first aid and emergency interventions) to improve client’s outcomes. Facilitate the triage process using clinical knowledge and appropriate interpersonal skills, demonstrating empathy, and understanding. Smooth implementation of community/facility-based ART, PrEP, MHPSS, Cervical cancer management, STI, and TB service to key population. Clinical assessment that determines the clinical urgency of the clients presenting health concerns and culminate with the allocation of care category, which determined the time and sequence in which they received services.
  • Documentation: client records are well maintained and updated. Complete documentation of client demographics and vital signs in the clients care card. Patient facility care and support form, adult clinical evaluation form and TB screening tool are always updated, documented, and completed. Proper documentation of the clients screened for STI and TB are in the register
  • Meetings: official OSS weekly meetings, monthly state program meeting, CMs and PNs weekly review meetings, Field workers review meetings, and any other meeting as directed.
  • Programs: Sustainable provision of quality support to HIV positive KPs clients on the SFH-KP-CARE-2-Project.Case finding and initiation of ART, PrEP, viral load optimization, tracking of defaulters, adherence counseling of positive clients, and syndromic management of STI and wound dressing at community during outreaches. Program supervisory functions to an assigned CCSAP.
  • Report writing: Weekly work plan, Monthly program report.
  • Report writing daily, weekly, monthly report.

Qualifications/Experience:

  • Must possess a university degree in Nursing or completion of an accredited nursing programme. Possession of a higher training would be an added advantage.
  • Minimum of 3 years’ experience working in public health programs, with a focus on HIV/AIDS prevention and treatment.
  • Must possess minimum of 3 years’ experience in nursing practice including the practice of triage.
  • Must possess good relationship management/ interpersonal skills.
  • Fluency in Hausa language in addition to English language and other local languages.
  • Knowledge of WHO and national HIV prevention, treatment, care, and support guidelines including the TB/HIV guidelines
  • knowledge of the ART regime and current global/national guidelines for dispensing ARV.
  • Knowledge of client-centered differentiated service delivery models for key populations
  • Experience in maintaining accurate computerized triage assessment records is an advantage.
